I fear that you can enter madness if you relax beyond a certain point, where you can feel unreal. Maybe that is the feeling of having an empty mind rather than madness, I have thought about learning meditation but the thought of having an empty mind scares me a little. Maybe that is being relaxed, something which is alien to me as I have been in a state of being constantly on guard and aware of my surroundings for so long.

Makes perfect sense - moving back into being relaxed after a long while can at times be a little bit like making a step into the unknown. However, we are not born into a state of anxiety and we all have the capability of being relaxed. There a major advantages in persisting and learning the concept of being relaxed - both physically and psychologically.

Your mind is never totally empty or devoid of thought. There's always something going on, the key thing is choosing not to focus or jump aboard those passing thoughts, and by doing so the number of thoughts that occur will reduce, and you can think instead about how pleasant it is to enjoy being relaxed.

When we are stressed we tend to scan the environment for danger. This is a perfectly natural process and something that we normally do at a low level quite unconsciously. When stress moves into anxiety we use our higher functions to give that natural scanning process a big helping hand amplifying any possible threat or danger. This is why a party-popper can sound like a gun shot, kids playing like a riot etc. As you begin to learn to relax these anxious excesses will gradually reduce.
