I am not sure whether I would put my problem down to anxiety or exam nerves, but I have just failed my driving test for the 5th time and although I can drive really well during my 1.5 hour lessons every week, as soon as I take the actual test it's like I have never driven - my head has nothing in it, I don't understand the instruction and once the exam is over I don't even remember the route we took or what I did during the exam.

I understand! Thankfully the solution is pretty rapid with a target for resolution of one extended session using NLP / hypnotherapy. The cost for the first session would be 80 pounds for approx 2 hours. Any additional boost sessions would be 1 hour in duration and cost 50 pounds. Any CDs / notes required would be provided free of charge. There are no hidden costs at all and our goal would be to attain resolution as quickly as possible. The issue itself is one of performance anxiety and is probably triggered by a remembered bodily sensation that has become linked with "driving test", your focus is probably on the sensations / associated thoughts and not on the driving test (a light trance state initiated by the freeze response). The process of reversing such associations is generally quite rapid.
